Description
It is the employer’s responsibility to designate leave as family and medical leave and to give the employee notice of that designation. These forms, designed for use in California, contain the information you need to designate leave as FMLA/CFRA or FMLA/PDL, to provide conditional approval of the request for leave if more information is needed, or to deny the request.
These forms contain additional information that must be communicated to an employee regarding your company’s approval of FMLA/CFRA or FMLA/PDL leave, including:
- Proper documentation of the amount of time taken
